What Is IMM 5476?

Form IMM 5476, titled Use of a Representative, is used to give someone permission to act on your behalf with Immigration, Refugees and Citizenship Canada (IRCC) and the Canada Border Services Agency (CBSA). This representative can correspond with IRCC regarding your application and receive information on your behalf.

When to Use This Form

Complete IMM 5476 when you are:

  • Appointing a representative
  • Updating contact information for an existing representative
  • Cancelling the appointment of a representative
  • Cancelling and appointing a new representative
  • Withdrawing your role as representative

Types of Representatives

Representatives can be either paid or unpaid. Unpaid representatives may include friends or family who do not charge for their help. Paid representatives must be authorized professionals such as members of the College of Immigration and Citizenship Consultants (CICC), lawyers or paralegals in good standing with a provincial law society, or Quebec notaries. IRCC will only work with paid representatives who are authorized.

How to Complete the Form

The form includes sections for:

  • A: Applicant information — personal details, UCI number, application type
  • B: Appointment of representative — representative’s name, status (paid/unpaid), contact information and declaration
  • C: Cancel representative — filled only if cancelling
  • D: Withdrawal by representative — if the representative is stepping down

Submitting the Form

You can submit IMM 5476 with your initial application if not submitted yet, or upload it later using IRCC’s secure web form. For paper applications, send the completed form to the appropriate IRCC office. Always inform IRCC of changes to your representative’s information.

Related Forms

To authorize IRCC to release information to someone who is not acting as your representative, use the Authority to Release Personal Information form (IMM 5475). This allows access to application status but does not give permission to conduct business on your behalf.
