State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
In January 2023 the secretary of health suspended the certified nursing assistant license of Franklin M. Van Doren (NC60413258) pending further disciplinary proceedings. The Department of Social and Health Services’ Adult Protective Services determined that Van Doren abused a vulnerable adult.
In December 2022 the Nursing Commission suspended the registered nurse license of Catherine M. Dailly (RN00167662). Dailly drank alcohol during her shift at a kidney dialysis center and failed to respond to the Nursing Commission’s request for a written statement.
In January 2023 the secretary of health and Kurt Allgood (CG60795582) agreed to an order that suspends his agency affiliated counselor license. Allgood sent messages to four co-workers asking for sex.
