What Is IMM 5406?

IMM 5406, titled “Additional Family Information”, is a supplemental form required for many Canadian immigration applications to provide detailed personal information about your immediate and extended family members. The form can be completed digitally through the Permanent Residence Portal or as a PDF for paper applications.

Who Must Complete the Form

The following people must complete IMM 5406:

  • The principal applicant;
  • The applicant’s spouse or common‑law partner (whether accompanying them to Canada or not);
  • Dependent children aged 18 or older, whether accompanying the principal applicant or not.

Form Sections Explained

Section A — Personal Details

Enter personal details for yourself (the principal applicant), your spouse or partner (if applicable), and your two parents. If married, indicate whether you or your spouse were present at the marriage ceremony.

Section B — Children Information

List all your children, including:

  • Biological children;
  • Adopted children;
  • Step‑children;
  • Children of your spouse or partner adopted by others;
  • Children in custody of another guardian.

Section C — Siblings

Provide personal details for your brothers, sisters, half‑siblings and step‑siblings.

Section D — Declaration

Read all declarations carefully and sign and date the form to certify that the information provided is complete, truthful and correct.

How to Complete and Submit

You can complete IMM 5406:

  • Online via the Permanent Residence Portal if submitting an electronic application;
  • By downloading and filling the PDF version if your application is on paper.

Ensure all applicable sections are completed fully. If a section does not apply, enter “N/A”.
